The time, the topic(s), your thoughtfulness and thoroughness are so important to me. The map is a fantastic tool for me to use as I continue to navigate expressing my wants, needs and desires around end-of-life issues. ~ Carla B.
Create a Map for the Future
An educational series in five key areas of life
for you alone or you and your family together to start the advance planning process |
Quick Start
The advance planning primer asks specific bold questions that you can begin to address now so you're more prepared later for what will come
|
|
MODULE ONE: |
CREATING COMFORT AND A HEALING ENVIRONMENT; CARE FOR THE BODY
We address the following: creating a home environment that is safe and adaptable; physical comfort; after death body disposition options. We also will begin practical action steps. |
MODULE TWO: |

We’ll complete a life review; discuss completing unfinished business; write a legacy statement and/or consider legacy gifts. We will continue with practical action steps. |
MODULE THREE: |
HONORING PERSONAL FEELINGS AND HEALING RELATIONSHIPS
We'll discuss any feelings around aging, becoming unwell/chronic illness, dying and death; discuss healing relationships. We’ll also create action steps for regarding personal possessions. |
MODULE FOUR: |
HONORING PERSONAL BELIEFS AND PRACTICES; COMMUNITY
We begin to cultivate a support system/community resources (spiritual or otherwise); define what is sacred; ancestral reflection. We will have an in-depth discussion regarding practical matters that arise as one ages. |
MODULE FIVE: |
PRACTICAL MATTERS; CARE FOR THE FAMILY AND PETS
Although practical matters have been addressed in each preceding module, we get into the nuts and bolts of the “toolkit” guide: care teams, advance directive; estate planning referrals; financial referrals; care for the family and pets. |
Can I do the series in my own time and on my own schedule? |
What electronic technology do I need? Will there be any writing? |
Yes. The series is a personalized ongoing process. We go at your pace. There is no set schedule other than what works for you for completing the six modules. You can schedule in advance or schedule as you go. As you begin to organize your life, some aspects of your journey may require more care, time, or healing.
|
The first session is a complimentary discovery session via phone. The following session is an information exchange and planning session for the modules that follow. The five module sessions are completed via Zoom. There will be pre-session reflective homework which can be completed on the computer or printed and filled out by hand.
|
Can I make payments? |
How do I sign up and when does the series start? |
Yes. At your discovery session, we will talk about a payment plan if that serves you best. You may cancel any time before the first module session is set; the series is non-refundable after the series of sessions commences. If you're unsure about the level of commitment needed for the series, consider the Advance Planning Primer. The cost of the Primer will be credited to the series if you decide to undergo the series at a later date.
|
To sign up, schedule your complimentary discovery session.
After our call, I will send you an Agreement and you can pay the deposit online. You will then receive a link to schedule your first appointment, which is generally about an hour long. In the appointment, we'll go through the series process and schedule. After the appointment, you will receive your first homework assignment.
